Site Certification (Non-DV) CEU Site Renewal Fee

Non-DV CEU sites are certified for one year and must be renewed annually.
Checklist of Items to include for the CEU Site (Non-DV) CEU site:
- Attach the Cover Form (ICDVP Form 10) and CEU Site (Non-DV) Renewal Form (ICDVP Form 10H) completely filled out and signed.
- Example of proposed CEU workshop including day(s) and time frames.
- Copy of training certificate or letter of completion. Certificate must state the agency is an ICDVP approved CEU training site.
- Annual fee of $150.
